The Prayer request form contain just about everything that a congregation would consider placing on a prayer request card. No need to add or modify the information contain on the card. Great job in creating such a great product.

These cards are freeing up a LOT of time for us. Prior to getting these prayer cards the pastor took verbal requests........very lengthy. We love them

These are great for what what we need at the church i attend. Prayer request as well as contact info on the reverse side for doing visitation prayer for follow-up. Don't know if i would change anything on it.

This 'Prayer Request' card is excellent quality and a great value. It also provides enough room to make a detailed request because it has 2 sides (no picture). This card will be our every Sunday prayer request card in the Church pews.
